CHAPERONE PARTICIPATION & MEDIA RELEASE

Life at Your School – Student Film Series (Life at Elsie Allen High)

This Chaperone Participation and Media Release (“Agreement”) is entered into by and between Faba Films, The Joy Lab, Inc., and the undersigned Chaperone in connection with the Life at Your School student film series.


1. Role & Purpose

The Chaperone is participating in this project in a supervisory and/or behind-the-scenes capacity, including but not limited to:

  • Student supervision

  • On-set support

  • Logistical assistance

  • Presence during on- or off-campus filming

The Chaperone understands that while their primary role is not on-camera performance, their presence may be captured incidentally in behind-the-scenes footage, documentary material, or promotional content. All chaperones will be named in the end credits of the final film.


2. Filming Details

  • Project: Life at Your School – Student Film Series

  • Filming Dates: Between June 15, 2026 and July 31, 2026

  • Locations: School campus of Elsie Allen High School, Santa Rosa, CA and/or approved off-campus locations

  • Premiere Event: August 15, 2026


3. Voluntary Participation & Right to Withdraw

Participation is voluntary.

The Chaperone, Faba Films, or The Joy Lab, Inc. may withdraw from this Agreement at any time, for any reason, without penalty. Withdrawal may limit future participation but carries no negative consequences.


4. Media Release & Likeness Consent

The Chaperone grants permission to Faba Films and The Joy Lab, Inc. to record, photograph, and capture their image, voice, likeness, and/or name as part of:

  • Behind-the-scenes footage

  • Documentary material

  • Educational content

  • Promotional or marketing materials related to the project

The Chaperone understands that:

  • Inclusion in the final project is not guaranteed

  • No additional approval is required prior to use

  • The footage may be edited, altered, or combined with other material


5. Distribution

The Chaperone acknowledges that the project may be distributed via:

  • Educational screenings

  • Film festivals

  • Free public streaming platforms

  • Promotional channels related to the film

No financial compensation is provided for appearance or participation.


6. Release & Assumption of Risk

The Chaperone understands that participation may involve typical risks associated with being present on a film set or school environment. By signing this Agreement, the Chaperone:

  • Assumes responsibility for their own conduct and safety

  • Releases Elsie Allen High School, Faba Films and The Joy Lab, Inc. from liability for ordinary accidents or injuries, except in cases of gross negligence or willful misconduct


7. Professional Conduct

The Chaperone agrees to:

  • Follow all safety guidelines

  • Respect student privacy and school policies

  • Act in a manner appropriate for an educational environment


8. No Employment Relationship

This Agreement does not create an employment relationship between the Chaperone and Faba Films or The Joy Lab, Inc.



9. Governing Law

This Agreement shall be governed by the laws of the State of California.
